COVID-19 ASSUMPTION OF THE RISKS AND WAIVER OF LIABILITY
As consideration for Notable Beginnings permitting me to take Music Together classes, I agree to the following:
ASSUMPTION OF THE RISKS: I understand that naturally occurring disease processes (including, but not limited to, the novel coronavirus or COVID-19), may exist in the community, including at Notable Beginnings, and present an inherent risk that cannot be eliminated. I acknowledge that, although Notable Beginnings is taking reasonable measures to avoid contact, transmittal, and exposure of disease processes like COVID-19 between people in our classes (including frequently sanitizing our instruments & equipment, Music Together Teachers, and third-parties), it is ultimately my responsibility to ensure that I and my child take appropriate actions to safe-guard ourselves. I understand and agree that as a registered member of Notable Beginnings, I am accepting and voluntarily assuming the risk that I or my child may be exposed and become ill as a result of COVID-19 and other diseases.
WAIVER OF LIABILITY: I, on behalf of myself and my child hereby voluntarily release, forever discharge, and covenant not to take legal action against Notable Beginnings, and its owners, managers, members, employees, agents, and volunteers (“Released Parties”) for any claims that may arise out of or relate in any way to my or my child’s exposure to any communicable disease, including but not limited to the novel coronavirus or COVID-19. The claims hereby released include, but are not limited to, claims of negligence against any of the Released Parties.

FAYETTEVILLE, I UNDERSTAND I HAVE GIVEN UP CERTAIN LEGAL RIGHTS AND THAT THIS IS A BINDING LEGAL DOCUMENT.
Here are the details:
* For Fall Bongos collection, Miss Lesley will be running 30-minute-long live classes at our standing class times through the web based platform called "Zoom". You and your children will be able to join the Zoom session up to 5 minutes early and stay up to 5 minutes late so you can "virtually visit" with me, your friends and your children's classmates for a bit before and after class.
* In addition, at your convenience, you and your children will be able to access one additional 20-minute-long on-demand class. Each on-demand class will have new songs and other new content.
* The classes are set to begin on September 9th and continue for 10 weeks.
* In addition, during this difficult time where many of us are homebound, Music Together will be providing downloadable coloring/activity pages, parent education printables, videos from around the world and other exclusives. This is unique for this "online" semester!
Families wishing to withdrawal from class must do so before the registration deadline for each semester (typically the first day of classes). Families are responsible for class attendance. Non-attendance does not constitute a withdrawal and all tuition fees are forfeited if the director is not informed of your withdrawal before the registration deadline. If circumstances necessitate withdrawal from class, please call the director at 724-949-0088.

The expectation of Notable Beginnings is that your child has been vaccinated in accordance with the recommendations of your pediatrician.
